Inattentional blindness, cognitively making choices without conscious perception. A lot of our perceptual processing occurs outside consciousness. We didn"t see what we should have plainly seen because attention is not focused on it. Eliminate, control or accept problem, but accepting problem is not a good idea and it"s hard to eliminate a hazard, so need to control it. Education information is the most useless thing for changing the system. You can"t just inform them about a problem, won"t necessary impact behavior. Need to know the hierarchy of effectiveness. How to find the information we need, when we need it. Rules and policies may just sit on a shelf, no one"s going to look them up unless they need it. Start at the bottom and work upwards. One of areas of biggest errors are omissions, where it"s likely something will get forgotten. If you want to tell people how to program an infusion pump, put a reminder note on there.
