Presence of muscle tone during sleep: not better explained by another disorder, causes, normal rem sleep, atonia of skeletal muscles. "idiopathic" rbd - can"t diagnose with any other disorder: problems with rem control. Idiopathic rbd = pre-symptomatic synucleinopathy: early on in their disease course - important to study them, trying to figure out which drug to test. Lesions of limbic cortex may also result in rbd: limbic encephalitis, voltage-gated k channels, anti-ma2 paraneoplastic, rbd is dangerous, potentially lethal behaviors, treatment of rbd, physical measures, remove dangerous objects from around the bed, hospital bed with bedrails. Soothing alarm: consider changing antidepressants to wellbutrin, not a great antidepressant but reduces rbd, medications, clonazepam. It"s a sedative - but parkinson"s already has gait and cognitive disorders, so it"s not a good option to use on them. A small benefit in a small dose (3 mg)
