Cognitive - Directed attention task-appropriately

We are usually somewhere in the middle of the normal distribution and we can engage in activities
without facing a lot of distress, impairment, deviant
There is no clear bright line between when you're functioning well between the domains or not, it's
a very subtle gradient
Criteria for Diagnoses:
Distress:
Some form of subjective distress
Wanting to form relationships but are terrified and are not able to do so regardless of
multiple attempts is distress
Mania - feel a manic state but it elicits a sort of euphoria which wants them craving for
more
The problem with this being a sole criteria is that sometimes a patient might be
subconsciously distressed and not aware of it or enjoys the mania, distress of hunger without
any disorder
Impairment:
Do things that are physically dangerous to themselves, start affairs, hurt themselves
emotionally
Problem with sole criteria is that people might have severe anxiety but show no signs of
physical impairment that is noticeable
Impairment does not have to go along with mental illness - anorexia nervosa does not
show disability
When you put impairment and distress together, then you can notice a pattern of
mental illness
Atypical/Deviant:
Dysfunction is when an internal mechanism is unable to perform its natural function
Using this as sole criteria, behavior is different based on culture. Also, people who defy
the social norms might not be mentally ill
Our beliefs about atypical behavior change over time - homosexuality was believed as a
mental illness until DSM 3
It is a very dangerous practice to engage in to decide what is normal or not
Then why is it included? It can offer important clues to show a sort of erratic behavior
out of nowhere - like somebody who was never religious but suddenly discusses about talking
with God or if there is a pattern of causing harm to others
Psychological disorder - Social, cognitive, emotional, or motivational dysfunction that is unexpected
in its cultural context and associated with present distress and impairment or increased risk of
suffering, death, pain or impairment
Donald anxiety case - there was distress because he thought he had lost out on 20 years, couldn't
drive
Kathy - not just poor judgement because it wasn't one, there is a pattern of addictive behavior, tried
to burn the house down, was very unhappy with everything
