Failure of a couple to conceive after 12 months of unprotected sex. For female partners= 35+ and older after 6 months. Fecundability- ability to achieve pregnancy in one menstrual cycle more accurate measurement to evaluate fertility potential. 20-25% fercundity rate for unprotected sex couple for the first 3 months. 80-90% are able to spontaneously conceive within 12 months. Oocytes aging- number of viable follicles decline. Age of loss accelerates after mid 30s. Oocyte age is the single most important factor affecting the probability of success with art. Measures number of follicles (2 to 10mm in diameter) between days 2 to 4 of menstrual cycle. Presence of 4-10 is sign of good reserve. During ovarian follicular phase, several antral follicles on ovary > 5mm in diameter. Follicle selected to develop into dominant follicle in response to fsh. Dominant will grow at rate of 1 to 3 mm/day till it reaches on average 22mm, then rupture.
