A health problem is a condition that necessitates intervention to prevent or resolve disease/illness or to promote coping and wellness. Nursing diagnoses actual or potential health problems that can be prevented or resolved by independent nursing intervention. Formulating and validating nursing diagnoses page 262-265: when the nurse recognizes a cluster of significant patient data indicating a health problem that can be treated by independent nursing intervention, a nursing diagnosis should be written. What is not a nursing diagnosis page 269-270: medical diagnosis, medical pathology, diagnostic tests, therapeutic patient needs or nursing goals, a single sign or symptom, an un-validated nursing inference.
