More adhesion molecules are present, monocyte recruitment rate is higher, ses (sub- endothelial space) is larger, ec turnover is higher (cobblestone appearance) Retention of ldl increases once it is in place. Intima layer is broken passing materials enter the ses. Immune system responds damage control: white blood cells (monocytes) are sent in; make the situation worse. Sent to the site of the lesion: attracted by ldl (especially oxidized), mcp-1, activated monocytes will phagocytose ldl and oxidize it. Foam cells accumulate and will distort the ec (endothelial cell layer), pushing out into the lumen. Ec gets thinner, allowing more materials to flow into the ses. Muscle cells and connective tissue will infiltrate.
