Interprofessional: patient safety and healthcare quality: the institute of medicine (iom) asserts that ineffective communication in healthcare systems can cause injury/death. Therefore, the iom recommends the use of sbar: electronic health record: legal documentation that allows for communication between the healthcare team and is a source of information.
