May benefit from supervised exercise training vs. unsupervised. Primary mode of type of aerobic exercise is walking to help build collateral circulation. Goal is 30-60 mins. /day of walking, but may need to initially start with 10 min. bouts accumulating to 30-60 mins. /day. Some patients are so deconditioned and debilitated by the disease that they must start the program only accumulating 15 mins. of aerobic exercise/day, gradually increasing time by 5 mins/day biweekly. Pre and post exercise bilateral ankle-brachial index (abi) may be assessed as part of a supervised exercise program. Cerebrovascular accidents (cva) and stroke occurs when a blood vessel in or leading to the brain is obstructed. Functional, sensory, emotional and cognitive impairments result. Main goal with supervised exercise programs = return the patient to safe and functional adls. Bike and semi-recumbent steppers (nustep) are best modalities for this population. Treadmill walking can be considered if the patient"s balance is sufficient.
