Complicated lesion: complicated lesion, rupture of the fibrous cap, thrombus formation with gradual increase in size, rupture of plaque starts clotting cascade. Non-modifiable risk factors: increasing age, gender, ethnicity, men more common until age 75, more common caucasians, genetic predisposition, familial hypercholesterolemia (autosomal dominant) Modifiable major risk factors: elevated lipid levels, elevated ldl, elevated triglycerides, low hdl, hypertension (> 140/90, tobacco use, physical inactivity, obesity (waist circumference 40 in men and 35 in women) They have an ldl goal of <70 mg/dl (1. 8 mmol/l). Risk factor: hypertension: defined as a bp >140/90 mm hg, for people > 60 years, a goal bp of < 150/90 mmhg is recommended (jnc8 recommendations) to prevent stroke, Stage 1 hypertension is 140 to 159/90 to 99 mm hg. Stage 2 hypertension is bp > 160/100 mm hg: those with stage 1 or 2 hypertension often require more than one drug to reach therapeutic goals.
