Dysmenorrhea: term for painful or difficult menses, often associated with hormonal imbalance. Perimenopausal period for most women occurs between ages 42 and 58 years. Common symptoms: hot flashes, excessive sweating, palpitations, vaginal dryness, painful intercourse. Endometriosis: when uterine tissue grows outside of the uterus. Leiomyomas (fibroids) uterine tumors can become quite large and multiply. Para = number of pregnancies that reached 20 weeks gestation. Inspect skin and hair over mons pubis and inguinal area for distribution patterns, lack of skin integrity. Inspect and palpate labia: majora and minor and clitoris for any pigmentation or surface alterations. Inspect urethral meatus, vaginal introitus and perineum for anatomical positioning and changes to tissue surface. Assess perianal and anus for color and changes to surface tissue. Not usually performed by a nurse unless advanced practice training has occurred. Patient may be standing or lying down. To evaluate for hernia have patient stand up. Explain what you will do before you do it.
