Physical Fitness and Stoke
ACLS Study
What else can we do to identify causality?
Not feasible to do RCT examining the effect of exercise on disease endpoints (CHD,
stoke, death)
However – we can look to see if exercise influences known CVD risk factors
Lee & Blair (2000)
Compared with the least fit quintile (#1)
Quintiles 2 and 3 had 0.37 times the relative risk of stroke mortality
Quintiles 4 and 5 (most fit) had 0.32 times the relative risk of stroke mortality
However only 32 cases of stroke mortality
were documented. N’s are very small:
14 (low fitness) vs. 11 (moderate fitness) vs. 7 (high fitness group).
Blood pressure: results from RCTs
Meta-analysis of 54 RCTs (only difference on groups was aerobic exercise)
Average reduction in systolic BP: 3.84 mm Hg
Average reduction in diastolic BP: 2.58 mm Hg
Reductions were noted in hypertensive and normotensive; and in overweight and normal
weight participants*
Note that this is what was found in MOST studies – not all studies found the same thing

Document Summary

Not feasible to do rct examining the effect of exercise on disease endpoints (chd, stoke, death) However we can look to see if exercise influences known cvd risk factors. Compared with the least fit quintile (#1: quintiles 2 and 3 had 0. 37 times the relative risk of stroke mortality, quintiles 4 and 5 (most fit) had 0. 32 times the relative risk of stroke mortality. However only 32 cases of stroke mortality were documented. 14 (low fitness) vs. 11 (moderate fitness) vs. 7 (high fitness group). Meta-analysis of 54 rcts (only difference on groups was aerobic exercise) Average reduction in systolic bp: 3. 84 mm hg. Average reduction in diastolic bp: 2. 58 mm hg. Reductions were noted in hypertensive and normotensive; and in overweight and normal weight participants* Note that this is what was found in most studies not all studies found the same thing. Hypertension is another major risk factor for cvd which is influenced by exercise.
