Periodontal risk factors and modifiers: periodontal tissues, organ, constant presence of microbes. Infectious diseases: pathogens and susceptible host together, periodontal disease, biofilm + susceptible host = host modifiers --> anatomical event, periodontal diseases. Immune system acts against bacteria and its products: destruction of host tissue. Lower implant success rate and more implant complication in smokers: diabetes/obesity, type 1 and type 2, risk factor for severity, not extent of periodontitis, controlled diabetes do not show an increase in extent and severity, mechanisms. Increased collagen breakdown: reduced immune response, deposition of advanced glycation end products. Il-1 gene polymorphism and chronic periodontitis: 65% of patients with severe chronic periodontitis exhibited polymorphism in il1 gene. Il1 gene polymorphisms lead to more expression of il1 locally (gcf) and systemically causing more inflammation: risk modifiers.
