An infant with less ability or chance to survive, or increased risk of permanent handicap. Risks: high risk mom, premature, rds, sga, iugr, post term, sensitization problems, birth injury, congenital anomalies. Born before the organ systems have matured to the point of physiologic functioning. Late pre term: ga 34-36 6/7 weeks. Associated with: low socioeconomic level, multiple births, pih in mom, poor prenatal care, placenta abruption/previa, mother < 17, cigarette smoking, previous hx of early birth, cardiac, rh sensitized, or diabetic mother, grandmultipara. Cardiovascular system: heart action slow, feeble, decreased clotting factors. Gi system: lose large amt of weight. Physiologic jaundice occurs earlier and is more severe. Ability to filter toxins is not fully developed. Defense mechanisms: natural defense mechanisms are poor. Nursing care: maintain resp function: open airway, give o2, maintain warm stable environment- warmer, isolette, use skin probe for continuous temp montioring, maintain nutritional state: small freq feedings.
