The nurse should have correctly and accurately recorded in the emar each time the physician requested medication. When the nurse received verbal orders she should have read back immediately upon receiving and documented that this was done. The nurse could also have called the physician back if noted orders have not been signed yet and again document this was done. With a documented trail the nurse has grounds to stand on and proof that she did everything correctly.
