Its elements are created separately and then combined. Filmmakers practice an art of elegant misdirection. Creating a sense of wholeness, completion, continuity etc. from things that do not actually exist. A matte is a mask that blocks part of the image and prevents exposure. The earliest mattes and composite shots were done in-camera. Painting is done on a pane of glass to create a background with paint scratched away where action takes place.
