Definition it is an infective gangrene of the subcutaneous tissue due to staphylococcal (staphylococcus aureus) infection, sometimes gram-negative bacilli and streptococci also causes carbuncle. Sites carbuncles are mostly seen on the back, in the nape of the neck and shoulder where the skin is coarse and vitality of the tissue is less. The initial lesion is similar to a boil in the form of hair follicle infection, since majority of patients are diabetics, infection takes a virulent course and results in necrosis of subcutaneous fat which gives rise to multiple abscesses. These abscesses are intercommunicating and they open to the exterior by multiple openings which are called sieve-like openings. Complications : extensive necrosis of skin overlying carbuncle can cause acute infective gangrene, septicaemia. It must be remembered that incision is never made unless there is softening in the centre. Technique a large cruciate incision is made extending upto the margin of the inflammatory zone.
