Feelings of apprehension, tension, uncertainty, dissatisfaction, and fear. Anxiety disorders: panic disorders, phobias, ocd, generalized anxiety. Treatment: psychotherapy, behavioural intervention, medication: sedative hypnotics. If anxiety is secondary to another disorder (like psychosis) treat primary illness first. Produces calming effect, relief of anxiety with little/no effect on motor or mental function. Higher doses of these drugs produce this effect. Phase 1: oxidation by p450, all metabolites active (long duration) Bind some gabaa receptors at site other than gaba binding site. Frequency of gaba-mediated opening of cl- channel inhibition of many cns neurons. Cannot open channel on its own (requires gaba for effect) Metabolites generally not active but can expression of p450 (enhance their own metabolism, need to take higher dose subsequently to have same effect) Bind all gabaa receptors at site other than gaba and benzodiazepine binding site (steeper dose-response curve because they bind all subtypes) Can directly open channel themselves without gaba there at high concentrations.
