• Start day -7. One fraction daily (2.4Gy)240 c Gy for 5
consecutive days(d-7till d-3 ) day --/--/20-- , --/--/20-- , --/--

• Premedicate with granisteron 1mg 1hour prior to TBI
+20mg decadron
• Lorazepam 1-2mg PO one hour pre-TBI
• TBI must be completed prior to HSCT infusion
Etoposide:
On --/--/20-- (day –2) etoposide ----gm (60 mg /kg ( adjusted
BW ) IV PB over 4 hours ( provided undiluted) (9AM) Flush with
100ml normal saline post administration
