Medication Administration: Parenteral
Parenteral Medication Concepts
Bypass GI system
It cannot be removed or retrieved once it is in the body- more dangerous medications
Safety first- be very careful
Requires sterile equipment and medication
Essentials to know
Site landmarks
Maximum volume that can be safely given at that site
Equipment to deliver medication to the site
Equipment
Sterile procedure. Follow sterile technique
We are breaking through a pts first line of defense (skin) and giving an injection
Diameter of needle= known as a gauge. 14 gauge is very large and wouldn’t used on
human
25 gauge is a very small and thin needle. Don’t hurt as much
IM is a 3ml syringe
Larger number = smaller needle diameter
21 gauge for IM is because fluid is more viscous or dense
Syringe size
Smaller volumes for injections up to 3ml
Syringe can come with or without needles. Need to come sterile wrap with expiration
date labeled. They are usually packaged separately
Intradermal injections
10-15 degree angle.
Very small amount
5/8th of an inch.
25 gauge
0.1-1.0 ml
Inner aspect of the forearm
Use Tuberculin Syringe holds up to 1 ml
PPD common example. Allergy testing
Just under epidermis so slower rate of absorption
Subcutaneous injections
45-90 dregres angle, needs to stay in subcontanous tissue. We don’t want it in the IM
space
We would use a 1-3ml for SQ but wit hinsulin we would only use an insulin syringe
(orange cap and written in units)
Small amounts: less than 1.5 ml (p.419)
Syringe varies by medication
Insulin syringe only for insulin
May be a 1 ml syringe or a 3 ml syringe
