For patients, period of transitions whether short or long can represent a time when multiple sources of distress are present, multiple needs exists, and multiple tasks must be accomplished. Transition can be improved if it is actively managed. Transition is passing or passage from one condition, action or place, to another . The concept always implies a change or movement. This stage is marked by disorientation, disintegration and discovery (liminality or neutral phase: stage three: the last phase or new beginning phase, where the individual finds meaning to the new reality and a new future has evolved. 4 main types of transitions: occur at all levels: developmental transitions, situational transitions, health-illness transitions, organizational transitions. Poorly managed transition processes have been shown to have a negative effect on the patient undergoing the transition whereas well organized transition processes actually benefit the patient.
