The hp program must have the highest level of support of the community that the program is being planned for because they can provide necessary resources for the program. Top-level people may have ideas for hp programs, but it doesn"t happen frequently. The idea could start with a variety of members in or outside of the community (ex. Employee in provincial health department providing consulting to employee in local health department)
