Goal is to control inflammation/pain within 5-7 days. Eliminate symptoms so patient return to routine life (decrease impact on quality of life) within 1-2 weeks. Goal is to lower uric acid levels to prevent precipitation, acute attacks, and formation of tophi. High risk of attack within 2 years and with time, it can become more frequent, so we want to prevent recurrence of flares overall. Treating/termination acute gout attack is not the same as preventing recurrence of acute gout attacks. Stop drinking alcohol & eating purine rich foods (fatty foods, frozen foods) Start eating vegetables, decrease meat/seafood portion per day, try reducing weight, exercise, drinking more water (2l/day) Can go with either colchicine or nsaids (acetaminophen, naproxen) Educate of how to prevent recurrences- diet, exercise. Explain that relief may not be immediate, but gradual. Explain that pain reaches maximum levels within first 12-24 hours so that"s why her medications may not seem to help her.
