Normal: sternomastoid and trapezius should be equal size. Equal strength should be seen when forcibly rotating the persons head to either side. Shrugging should have equal strength with resistance. Normal: no tremors or wasting of the tongue. Abnormal: tongue deviates to one side with lesions of the hypoglossal nerve. Normal: assess muscle size, muscles should be equal and a difference of 1 cm or less is normal. Abnormal: tics, tremors, fasciculation, myoclonus chorea and athetosis. Normal: observe as a person walks 3-6 m. the gait should be smooth and rhythmic with the opposing arm swing coordinated. Tandem walking (with feet directly in front of each other each step) should not cause imbalance. Abnormal: still immobile posture, lack of arm swings, unequal rhythm of steps, scraping of toe and shoe. Ask a person to stand still with their eyes closed. They should not lose balance after 20 seconds although swaying may occur.
