Pre-referral intervention: provides immediate instructional and/or behavioral assistance: responsiveness to intervention (rti triangle) Evaluation and identification: all children suspected of having a disability must receive a nondiscriminatory multi-factored evaluation. Program planning: an individualized education program must be developed for children identified as having a disability. Placement: the iep team must determine the least restrictive educational environment that meets the student"s needs. Progress monitoring, review, and evaluation: the iep must be thoroughly and formally reviewed on an annual basis. Greater proportions of students from diverse groups are born to mothers without access to maternal health care and live in poverty. Students" disabilities may be overlooked entirely because of their membership in a racial or ethnic minority group resulting in their being denied access to needed special education services. Direct and frequent measures of student performance provide the most meaningful information about a student"s progress and effectiveness of instruction. Annual review: the iep is not a stagnant document.
