Goals expected behavior or response that indicates resolution of a nursing diagnosis or maintenance of a healthy state. End results that is measurable, desirable, and observable: measure that tells you if the interventions applied led to successful goal achievement, nursing sensitive outcomes. Measurable patient or family state, behavior, or perception, largely influenced by and sensitive to nursing interventions: collaborate and evaluate effectiveness of interventions. Assessment skills and techniques and are used to determine if known problems have remained the same or changed. Five steps to follow when doing evaluation: examine outcomes criteria to identify exact desired patient behavior or response, evaluate patie(cid:374)t"s a(cid:272)tual (cid:271)ehavior or respo(cid:374)se, compare established outcome criteria with actual behavior or response. Judge degree of agreement between outcome criteria and the actual behavior or response. Why did it not agree: document results.
