Drug given iv goes directly to plasma (1st compartment) Drug is absorbed to blood & then is absorbed to tissues reaches equilibrium b/w tissues & plasma, but concentration in plasma is constantly going down as drug is used. Vancomycin is not absorbed so it is given iv unless it is for c. diff, then given orally. Fluoroquinolones are very useful for systemic infections but this has led to their overuse & resistance. When switching antibiotics from iv to oral (iv to po switch): Equivalent: if it is fully bioavailable then equivalent dose is given (ex: Parallel: when switching from iv drug to a similar, highly bioavailable drug that is different is a parallel switch (ex: ceftriaxone to bactrim) Step-down: oral dose & iv dose are very different (ex: ampicillin to amoxicillin)
