Identifying features:
- Carabelli cusp may be present.
- Rhomboidal Occlusal outline.
- MP cusp well developed.
- Prominent oblique ridge.
- D, B and P grooves.
- Crown similar width MD and BP.
- 3 roots widely separated – 2 B and 1 P.
- Largest teeth in maxillary arch
- 5 cusps: 3 major, 1 minor, 1 cusp of carabelli
- Broad M-D and just slightly wider B-P than M-D
- 2 buccal 2 palatal 1 carabelli
- 3 roots, 4 pulp horns

Cusp of carabelli from mp cusp: p developmental groove extends from the centre of the p surface between the 2 palatal cusps, where it curves to the distal side and becomes the distal oblique ridge. These 2 grooves (p and d oblique grooves) are sometimes considered to be one dp developmental groove: dp cusp is small (40% of p surface). Proximal mesial: shows a clear profile of cusp of. Carabelli: bp measurement on the mesial side is greater than that on the distal side.
