Inflammation of joints, not a disease just an umbrella term, disability and reduced quality of life (elderly), increased prevalence of arthritis increases with age. Primary manifestation is synovial inflammation = synovitis erosion of bone, cartilage, peri-articular structures. Incidence = 98 cases per 100,000/annum: prevalence 0. 5 to 1. 0% Occurs 2-3x more commonly in females than males. Peak age of onset 40 (range 40-70) years. Insidious onset over weeks to months (55-65%) morning stiffness of >1 hour, pain swelling. Acute onset peaks within a few days (8-15%) Distribution of joint involvement: symmetrical, commonly affected joints, upper limbs metacarpophanlangeal (mcp), proximal interphalangeal (pip), wrist, lower limb metatarsophalangeal (mtp) Can be prevalent in larger joints without major symptoms because more space: characteristic deformities e. g. boutonniere, swan neck, z, mcp subluxation and ulnar deviation, radial wrist deviation. Fixed flexion deformity of the proximal interphalangeal (pip) joint. Fixed hyperextension of the distal interphalangeal (dip) joint.
