Muscle wasting and weakness is caused by a variety of diseases/situations including cancer, Copd, diabetes, denervation, heart failure, disuse atrophy, muscular dystrophies, sarcopenia, chronic kidney disease and injury. Cancer cachexia = multifactorial syndrome characterised by ongoing loss of skeletal muscle mass +/- loss of fat mass that leads to progressive functional impairment. Cachexia is a multi-organ syndrome tumour release factors affect different parts of the body (appetite centres, adipose tissue) More common in children and elderly, increases with cancer. Present upon diagnosis in 85% of patients with gi cancers, 83% of patients with pancreatic cancer and 60% patients with lung cancer. Most cancer deaths are associated with cachexia (20-30%) Have to consider weight loss as well as smi or bmi. Spectrum of cachexia: precachexia = weight loss <5%, anorexia, metabolic change, cachexia = weight loss >5% or bmi <20 and weight loss >2% or sarcopenia and weight loss >2%.
