Case: Paediatric Orthopaedic Clinic at the Children's Hospital of Western Ontario

• Long wait times experienced by young patients, aggravating children's distress
• On average, patients spending roughly 2 hours in the clinic
• Staff also over-extended, yet ineffective utiliaztion of staff

The Clinic
• Jointly managed by the Children's Hospital of W O and the Division of Surgery, both a part
of the London Health sciences Centre
• The clinic operated from 8:30 am to 1pm and accepted only patients with prior
appointments.
• On atypical day, 80 patients pass through the clinic; 40 per cent are new patients and 60
per cent are follow-up patients.
• The follow-up patients can be further categorized into patients who require X-ray and
those who do not.

Step 1: Finding the ctivity utilization at every step in the process
What is the utilization for the:
• Nurses at the Front desk
• Technicians at the radiology department
• Surgeon in the examination room
Where is the bottleneck in the process? What are the capacity constraints at the clinic?
• To find the bottleneck, you have to find the utilization of the 3 types of workers.
• Use Utilization = Flowrate/Capacity of the system
