Insulin resistance leads to hyperglycemia; consequently glycosylated derivatives, reactive oxygen species and modified ldl damage the endothelium; macrophages and cytokines promote inflammation in the arterial wall. Most deaths in type 2 diabetes are due to coronary heart disease. Peripheral ischemia can injure the heart, kidneys and retina, and the small peripheral nerves are particularly vulnerable. Obesity is the biggest risk factor for type 2 diabetes, and obesity during childhood is particularly harmful. Epidemiology ( framingham study): obesity, smoking, hypertension, dyslipidemia, sedentary lifestyle (lack of exercise), diabetes type 2 the risk factors are additive.
