Imagined division reflects a continued desire to moralize addiction. Doctor shoppers go from doctor to doctor for prescriptions due to lack of records. Script docs doctor that easily gives out prescriptions, physician gets paid for number of patients and more money as they are known to prescribe drugs. Boredom very rural and lonely communities and something to do, disconnected from major routes of trade and makes drugs more desirable/exotic, blue-collar work becomes gateway to prescription (mines, steel, agriculture), tough, stressful lives and very poor.
