Document Summary

Plantar surface of joint capsule is thicker (not the same all around) plantar plate. Hallicus (big toe) flexor hallucis longus (muscle tendon complex) Flexor tendons cross the bottom of this joint. Mtp joint is extended (cramping usually with flexion) Failure on bottom of mtp (open up bottom surface) rip open. Added weight, force and energy (as if sitting on heal: axial load on foot that pushes more into extension, not all stories will be the same, toe just has to go into hyperextension. Flexible footwear (field cleats) especially in toe: allow extension of hallicus. Synthetic playing surfaces: cleat to turf is stronger (grip) than naturally would, as if ground grabs shoe. Body weight: more weight = more force = more injury. Roms affected: flexion/extension, flexion will not reproduce symptoms, active extension reproduces mechanism aggravating, passive can push farther into extension and cause more pain. Activity of daily living: jump landing causes extension while landing on toe.
