When pulmonary system is compromised: the bronchial supply increases this causes enlargement of the bronchial vessels, increased blood pressure or erosion by bacterial agents rupture of enlarged vessels blood spills into respiratory tree hemoptysis. The major source of massive hemoptysis is the bronchial circulation: the pulmonary circulation account for 5% of massive hemoptysis cases. Ranges from minimal blood in sputum to >500 ml of blood in sputum.
