Rettew et al: meta-analysis of agreement between clinical diagnosis and research diagnosis. Agreement (kappa) low to moderate for most disorders. Kappa= . 14 (affective and bipolar disorders) very poor. Samuel (2015): survey of studies comparing reliability of personality disorder diagnoses. Its a label for symptoms not causes and meanings for the disorders! Symptoms impair social, occupational, or other domains of functioning. Impairment by itself is not enough to suggest someone has mental disorders ex: Suicide bomber still mentally sound but impaired themselves. What about things that disturb us that don"t seem pathological? ex: very very shy but no label of mental disorder for it even if you"re experiencing personal distress. No clear line between normalcy and disorder. Abnormality" based on agreement of psychologists and psychiatrists. Pressures to include symptoms as disorders in dsm # of conditions in dsm just so the treatment gets paid for! Dsm is not a standard of mental illness, just guidelines!
