Wound: creates an alteration and disruption of the skin. - can be created: intentionally (surgical incision, accidentally trauma (motor vehicle crash, chronic forces (pressure injury/ ulcer) Wound repair/healing process (moisture balance, pt may also need vitamin supplies (cid:524) When caring wound, refer to function of skin to assist you to choose the best approach to care. It starts upon injury (no matter size of cut) Body immediately begins process of restoring integrity & physiologic functions of the skin. Most wounds heal by connective tissue repair (scar leaves a tunnel- by connecting tissue tgt) Three phases in healing process of wound: initial (inflammatory) phase: 3-5 days, granulation (proliferative) phase: 5 days-3 weeks, maturation (remodeling) phase: several months or years, initial phase: inflammatory/hemostasis. If intentional for surgery: surgeon will assist to protect skin from contamination so quick. Lasts for several days (or quickly depends on severity of wounds, contamination) Occurs immediate after injury vascular & cellular events occur.
