Is based on 6 criteria"s: scored 1-4, one being completely limited, 4 being no impairment, ranked on, sensory perception, moisture, activity, mobility, nutrition, friction and shear. Fowler"s position: 45 degree angle, support under the head, arms, and ankles. Supine position: pillow supporting the hear, wrists and elbows. Prone position: on stomach with support under ankles or hip, depending on patients preference. Side lying: pillow under head, hugging pillow, and under legs for support lateral position lateral flexion of the neck, spinal curves out of normal alignment, shoulder and hips are internally rotated, adducted, or unsupported. Raise the side rail on the side of the bed opposite to you. Elevate the level of the bed to your hips. Assess the patients mobility and strength to determine what assistance the patient can offer during transfer. Determine the need for assistance from other health care professionals.
