Mental status examinations with older adults, nse 13 week 8 readings. Setting: nurse at a long term care facility. Client: agnes admitted to a ltc for short term to determine if she is able to return home. Objectives: establishing trust, active listening skills, empathy, assessing client"s status. Mental status: inferred through individual"s behaviour, consciousness language, mood and affect, orientation, attention, memory, abstract reasoning, thought process, thought content, perceptions. Nursing assessment: focus on the health and illness experiences. Mental status assessment: biographical data, reason for seeking care, past health, family health history, current health. Functional assessment: nutritional pattern, sleep patterns, activity/mobility, elimination, interpersonal relationships and resources, self esteem and concept, spirituality, coping and stress management, smoking, alcohol, drug use, gambling, home and environmental hazards. Expressing feelings through art: can be very expressive, can see the different expressions of their feelings, can demonstrate feelings and expressions, symbolizes the increases in changes as disorders change.
