Nse13 week 12 skin, hair, nails, and peripheral vascular. Nerves, sensory receptors, blood/lymphatic vessels and epidermal appendages (hair follicles, sweat glands) If a rash inspect all areas of the body. Inspect mucous membranes for lesions in the mucosa. Assess non threatening areas first such as nails. Look at outer skin then underlying structures such as skin folds in breasts abdomen and groin. Use braden scale for skin risk assessment. Normal: general pigmentation, symmetry and consistency, melanin provides ton, widespread colour change. Secondary under the skin (into the dermis) Sensory perception, moisture, activity, mobility, nutrition, friction/shear. The peripheral vascular and lymphatic system review. Pulses (temporal, carotid, brachial, radial, ulnar, femoral, popliteal, dorsalis pedis, posterior tibial) Drain deoxygenated blood and waste from the tissues and return it to the heart. They run parallel to the arteries and lie closer to the skin surface.
