Definition: working with others, sharing ideas, pursuit of a common goal. Is a
development of partnerships to achieve the best possible outcomes that reflex he
particular need of the patient, family or community. Work with others. And you tell
people who are involved in the care including the patient what they need to know.
Circle of care: people who need to know the information to do their jobs.
Kim’s theory of collaborative decision making in nursing practice: involved the
patient in the decision-making. Client centered care and informed consent is needed.
Patients had the capacity to influence their own care. We care with them. Kim’s
theory involves collaboration between patient and nurse.
Interporfessional education collaboration: not all health care providers were on
the same page. All health care providers must communicate.
There are four attributes needed for a successful interprofessional collaboration;
Values and ethics: they should line up, self-awareness they can take all the
attributes of ethics into teamwork and collaboration. Person centeredness.
Roles and responsibilities: awareness or what you do and what others do. It’s a
shared workload and you need to know what others are doing. you avoid
redundancy in teams by clarifying roles.
Communication: you can’t have collaboration without communication, listening
and talking, common language and avoid jargon, timely communication, everyone
feeling equally empowered to share their ideas, using technology to communicate,
making sure the patient knows what health literacy is.
Teams and Teamwork: people work in teams to get things done more efficiently.
Types of collaboration: nurse-patient, nurse-nurse, interprofessional, and
interorganizational.
Nurse-patient- setting up a care plan, working with patient and family,
community partnerships, etc.
Nurse-nurse mentoring. LPN-RN collaboration, shared governance, quality
improvement, dedicated information exchange (meeting, memos,email), etc.
Interprofessional- nurse and physiotherapist, nurse and dietician, they aren’t
mentoring you they are just coming to help you do something, rapid response
teams (ex. crash team which has RNs, physicians, respiratory technician, someone
to document (which is a student nurse can do)), ethics committees, patient care
handoff, disaster preparedness teams, etc.
Interorganizational- working with homecare ex. VON, between hospitals,
working with other research departments, working with a non-profit ex. heart and
stroke foundation. Can be large like international or can be small like discharging
to homecare, UN etc.
