Consciously observing patient to form an overall impression. Use all 5 senses with the exception of taste. Requires adequate exposure to each body part being assessed. Approach patient with a gentle, calm and easy manner. Use different parts of the hand depending on where data is being gathered. Moderate palpation (1-3 cm below the dermis) Deep palpation ( 3-4 cm below the dermis) Wear gloves if you anticipate contact with body secretions. *rarely used as this can cause injury to the patien t. Listening to body sounds through a stethoscope. Diaphragm is used for high pitched sounds. Higher quality stethoscopes have a mechanism to switch the bell and diaphragm to pick up sound. Collect urine at the start of the exam. Done on exam table or held by parent. Leave diaper on as long as possible. Leave most uncomfortable assessments until the end. Offer choices (e. g. want to listen to mommy"s heart first?)
