Iron can decrease the absorption of other medications: Third trimester (not enough time for response to oral therapy) More patients achieved target hgb (within a 4 week time period) with iv iron. Adverse effects occurred more often in patients receiving po iron. Note: no hypersensitivity reactions occurred in any patient. Iv iron sucrose (most studied) used in 9/11 studies included in the review. Injection site reaction, muscle cramps, shivering, flushing, peripheral edema, pruritus. Note: anaphylactic reactions are rare ( 1: 200,000) Maternal : vital signs prior to infusion & hourly until infusion complete. Fetal : fetal hr prior to infusion & with maternal vital signs during infusion. Other routes of administration: intramuscular (not recommended, painful, skin staining ) Delivery considerations : in hospital delivery, iv access, blood products ready, active management of the third stage of labour (amtsl) to prevent pph. Consider checking ferritin in patients at risk of iron depletion: Consecutive pregnancies with less than 1 year interval in-between.
