Antacids and alginates have the fastest onset for symptom relief (minutes), while. Ppis are the slowest to relieve symptoms (couple of weeks: antacids = onset for symptom relief is 5 mins, but duration of action is short (30-60mins) H2ras have relatively longer onset compared to antacids, but duration of action is much longer (4-10h) Ppis provide faster symptom relief than h2ras, but slowest onset, however when taken continuously, they allow for better symptom control. Antacids need to be dosed frequently, require large volume, and tastes bad: sold otc, so convenient to get for short term, but if gerd needs to be treated long term, then it may become costly. H2ras treatment requires once daily dose, and management requires as needed dose. Ppis once daily dose, can be more expensive than antacids (if not covered in a plan) Short term goals: improve quality of life within few weeks. Relieve symptoms (heartburn) within 1-2 days (few days 2-4 weeks)
