Prenatal hormonal gender: testosterone and anti-mullerian hormone in males but not females. External genital appearance - male: penis, female-vulva, assigned gender at birth. Gender identity - sense of self as male or female. Internal accessory organs - male: prostate, female: fallopian tubes. When physiological factors are consistent but ones sense of self is not the same as their physical characteristics: gender dysphoria. In children: at least 6 months of incongruence between one"s expressed gender and assigned gender, 6 of the following : Preference for wearing attire of opposite sex. Preference of toys, games, etc of opposite sex. Desire for sex characteristics that match their experienced gender: clinically-significant distress or impairment in social, school or other areas of functioning.
