Lumbar contusions, strains and sprains: muscle strains result from sudden extension action, with trunk rotation on an oxertaxed, nprepared or under developed spine, chronic strains may result from improper posture, excessive lordosis, flat back or scoliosis. Non structural scoliosis: no bony deformity, not progressive, can be treated, disappears on forward flexion, may be caused by: postural problems, leg length discrepancy, hip contracture, muscle spasm. Lordosis: increased curve in lumbar spine, increase in anterior pelvic tilt, relative hip flexion, asis is lower. Flat back: decreased lumber lordosis, increased posterior pelvic tilt, increased hip flexor length, patient appear stooped forward, difficulty standing up, asis goes up. Facets are part of the posterior column of the spine: the facet joint capsules ac as passive restraints against excessive lumbar rotation. Symptoms: non specific low back pain with deep achy buttock and thigh pain, never below the knee, worse in the morning, aggravated by rest and hyperextension, relieved by repeated motion into flexion.
