The plantar aponeurosis consists of 3 bands: lateral, medial, central. When toes are extended, plantar fascia/arch is functionally shortened as it wraps around each mt head. Plantar fascia functions like a muscle = it has a dynamic function. It is responsible for transferring the weight from the medial to the lateral side of the foot during the gait cycle as well as arch support/dynamic shock absorption. 1 in 10 will report medial heel pain. Usually pain is at the medial plantar tubercle of calcaneus (occasionally to in the mid portion) Slow onset - changes in structure with no inflammatory mediators present. Hallmark sign is pain during first steps in the am or after prolonged sitting. Pain lessens during ordinary activity but increases as activity intensifies. This is because it is stiff because it starts to heal since feet are plantar flexed with sleeping. With active people it may be more changes in training (fitt) than over pronation.
