Pain would be acute because it has not been going on for more than 3 months. Describing lesion- fluid filled vesicles, erythema, chart the size and the exact location so next person can see if there is any change. Document- count possibly, measure area they are occurring in. *****for integument be really specific in location and description***** Note if the person is in any pain: 0-10 scale. Look bilaterally and see if both sides of the body are symmetrical. Hypertension- blood clot concern: blood clots lead to strokes, strokes are caused by hemorrhage and blood clots. Smaller women are at higher risk for osteoporosis. Peripheral vascular assessment: capillary refill, pulses- femoral, dorsalus pedus. Movement: range of motion- extension, flexion, rotational, circumduction, active range of motion* assess this first, passive range of motion- when you do the movement for them. Look at joints above and below: ankle injury- assess range of motion of toes and ability to move knee.
