Human factor is a scientific discipline and understanding the interaction between humans and other disciplines. Theory is about developing theories to design or optimize human well being. No matter how strong the impulse is to care there are times when it might seem easier to do a mistake or deny that something has gone wrong and become defensiveness. Important for us to understand that there is a relationship to what we do and how we fit into systems. Every step has a chance for failure in the health care team. The holes are opportunities for a process to fail and slices can be defensive area. Each layer of the swiss cheese is a chance for potential to cause a problem or error.
