Case Study 8
1. A Very Sick Student
OBJECTIVES
This case presents an example of infectious disease affecting
the lung.
You should:
• Review the host's defenses against infection.
• Review the specific factors in this host that have failed and
led to pneumonia.
• Review the characteristics of lobar versus
bronchopneumonia.
• Review the complications of pneumonia.
We can classify pneumonias as lobar or bronchopneumonia,
but what other classification can be used? Which category
would this case fall into?
Don't forget your knowledge of the inflammatory response and
the major cell types involved (e.g. acutely - neutrophils).
History
A first year science student at the University, found himself
alone in his rented room recalling recent events. He arrived in
London from Newfoundland in August and after settling in,
dedicated himself entirely to his studies.
There had been a few upsets. His post-secondary education
grant application had been rejected in Newfoundland because
he was "out of province" and his eligibility for OSAP was being
contested and had not been approved yet. He decided to open
his Christmas hamper from home and was delighted to find a
Christmas pudding from Mom, and a bottle of 'screech' from
Dad. There were a number of cards including one from his high
school sweetheart that informed him she was breaking off

their engagement. He curled up into a sleeping bag on the bed
and started into the pudding and the screech.
He awoke on Christmas day feeling cold and shivering. He tried
to keep warm as he wrapped himself in a blanket and walked
around the room energetically.
As the day went on, he developed difficulty breathing
(dyspnea), a cough and a pain in the side of his chest. He also
experienced chills and sweating. He took two aspirins.
The landlady had spent Christmas morning with her
grandchildren. When she returned that afternoon she saw him
and took him to a hospital emergency department.
