Epidemiology of pd: average age at diagnosis = 60 years, direct costs associated with pd = . 8m, dependence in adls happens 3-5 years after diagnosis, mortality only modestly increased. Enhances activity in motor cortex: binds with 2 types of receptors, d1 = facilitates activity in direct pathway, d2 = inhibits activity in indirect pathway, overall effect facilitate movement. Lewy bodies: aggregation of synuclein, start in brain stem and ascend to cortex. Cardinal features of parkinson"s disease: bradykinesia, sti ness, postural instability gait and balance problems, gait, shu ing gait. Tremor: can be present in the extremities, lip, jaw or tongue, not present in head. Pill-rolling in hand: most pronounced at rest, presenting symptom in ~ 70% of people diagnosed with pd, can be exacerbated by anxiety, emotional excitement and stressful situations. Bradykinesia: generalized slowness of movement, major cause of disability, decreased ability to initiate movement, starts distally in upper extremities.
